The Cooper Green patient portal allows you to view important information about the care you receive here, including:
- Lab results
- Upcoming appointments
- Details on medications prescribed here
- Documents created by your healthcare team
Also, you can use the portal to communicate privately and securely with your providers and clinics and get answers about your care.
How do I register for the patient portal?
First, you must have an email address on file with Cooper Green, which should have been saved when you first enrolled as a patient. Upon request, an email message with a link to complete your portal enrollment will be sent to your email address on file. To request a registration email, please call 205-930-3200 or email patientportal@cgmhs.org. Patients cannot register without receiving this registration email.
If I’m already registered with another facility’s portal (such as UAB), do I still need to enroll in the Cooper Green portal?
Yes. Cooper Green’s patient portal is currently only for Cooper Green.
IMPORTANT
In February 2027, Cooper Green will switch to a new patient portal called MyChart. This single portal will contain your health records from Cooper Green, UAB Medicine, and UAB St. Vincent’s, so patients will no longer need to enroll in multiple portals. This also means that your Cooper Green provider will be able to see your medical records from UAB Medicine and UAB St. Vincent’s, which will help improve the care you receive.
