If you need a ride to your appointment, Cooper Green can help. Our Social Services department now offers free rides to all appointments at Cooper Green and for appointments at other facilities if your Cooper Green provider referred you there, some exceptions may apply.
How to schedule a ride
To schedule a ride, please contact the Social Services department at least one week before your appointment. You can also speak to your provider at your next visit about rides to future appointments. We use two transportation companies to provide the rides – Prolific and Reliatran.

Cancellations
If you need to cancel your ride, it must be done 24 hours before your scheduled ride. Failure to cancel in time may result in a loss of service.
