The Cooper Green Resource Center is here to support you with helpful services and resources that make it easier to access our care, improve your health and well-being, and save you money.
Our services include:
- Transportation assistance – We provide free bus passes and transportation services for eligible patients.
- Prescription assistance – We can help you obtain eligible medications at little to no cost.
- Application assistance – We offer free guidance on completing applications for various social service programs, such as Medicaid, SNAP, and housing.
- Home health & hospice care – We offer support services for enrolled patients who need in-home medical and/or and hospice care.
- Social service referrals – We can provide referrals or assistance with any other services you may need.
Do I qualify?
These services are available to all enrolled Cooper Green patients. If you wish to become a member, our team can provide information on what’s needed and connect you with our Enrollment Department to help.
