Patient Portal

The portal gives you access to your care team and allows you to see or change upcoming appointments, request a new appointment, submit prescription refills, view your care plan, view test results, request an enrollment appointment, and more.

If you are not currently enrolled with our portal, you can request access to the new portal by emailing patientportal@cgmhs.org. You must have an email address and an invite from someone at Cooper Green to use the portal.

How do I get access to the patient portal?

See your Enrollment Specialist or Patient Encounter Specialist (PES) at Cooper Green to provide them with your email address. You will receive an email from no-reply@patientportal.net inviting you to create an account. If you are not coming to Cooper Green soon and would like to enroll, email patientportal@cgmhs.org to share your desire to join the portal. A link will be sent to your email.

Why would I use the patient portal instead of calling?

The portal is simply another option to contact Cooper Green. It is also available 24/7.

When are my records updated?

Records are updated within a week of becoming available, with some items being even faster. If you have questions about test results in the portal, please message your provider. Keep in mind that you will likely see your test results before your provider has seen them. Normal results, for example, will be posted immediately, likely before your next conversation with your provider.

How long until I receive an answer to my message?

You will receive an immediate acknowledgement when you submit a message and a Cooper Green staff member will respond to you within two business days.

I am having trouble logging into the portal. What do I do?

I don’t have a computer. Why should I sign up?

If you have an email account on your smart phone, you can still access the portal. You may also download the app from your phone’s smartphone

Does anyone else have access to my record?

Unless you share your login with someone, only you will have access to your health records.

Does the portal show my medications?

Yes. The portal will list all of your medications in the “Health Record” section.

What if my information in the portal is wrong?

Email patientportal@cgmhs.org, describe the issue, and provide your name and date of birth. We will review and get back to you as soon as possible.

What if my email changes?

In the portal, you can update your contact information. On the home screen, at the top right, click on account, select Security, and then edit your email and/or password. 

What if I don’t receive an invitation?

Please email patientportal@cgmhs.org and let us know. Please provide your name and medical record number, which is on your Cooper Green card.

Can I request or download a request form to get my medical records through the portal?

Yes, you can download a request for records, but you still must come to Medical Records to present ID.

Will I be able to complete an enrollment application via the portal?

You will be able to submit the application and upload documents needed for enrollment. However, in most instances we still need you to come to your enrollment appointment to finalize and determine your discount, if any. In some situations we can enroll over video or phone, call 205-930-3377 if this is needed.

Can I upload my enrollment paperwork through the portal?

Yes, you can upload any documents needed. However, in most instances we still need you to come to your enrollment appointment to finalize the process and determine your discount, if any. In some situations we can enroll over video or phone, call 205-930-3377 if this is needed.
