Items to Bring To Your Enrollment Appointment

HOW DO I ENROLL AS A PATIENT?
You must be 18 years of age, a resident of Alabama for 1 year, and a resident of Jefferson County for at least 30 days. Becoming a patient, provides you with access to our full range of health care services.

Cooper Green Mercy Health Services accepts Medicare, Medicaid, and many third-party reimbursement plans. In addition, if you qualify for financial assistance based on your income, you will be entitled to reduced co-payments for all services, including prescriptions filled through our on-site pharmacy. To participate in the financial assistance program, you will be asked to provide certain documents that verify your residency, identify, and family income. Shelter Continuum Cards are welcome. More information about our financial assistance program can be obtain from our Enrollment Office on the first floor.
